What we offer
Community First Services and Supports (CFSS) is a Minnesota Medical Assistance program that helps people with disabilities and older adults get the daily support they need while staying in control of their own care. Instead of fitting into a one-size-fits-all program, you decide who helps you, what tasks they assist with, and how your support fits into your routine.
Our team works alongside you and your family to build a plan around your goals, preferences, and strengths. Whether you need a few hours of help each week or more hands-on daily support, CFSS gives you the flexibility to live safely and independently in the place you call home.
Self-directed care
